To My Valued Patients:

An unexpected change in my personal life has led me to make the very difficult decision to close my practice. While seemingly sudden, I assure you this decision was the result of careful contemplation.

The last day that I will see patients will be April 30th, 2024. I will continue to provide medical care for you prior to this closing date however, I encourage you to begin efforts to establish with a new provider as soon as possible. Please complete an authorization form and return to our office to have your medical records forwarded to a new health care provider of your choosing. The selection of a new provider is your choice, and we are happy to send records wherever you decide.

Prescription medication refills will continue until May 31, 2024 and up to a 90 day supply will be granted if appropriate. After May 31st, 2024, no further refills will be granted.

I am including the contact information for Patrice O’Neill-Wilhelm, APRN, an experienced primary care nurse practitioner whom I have had the pleasure of working with for many years. She is currently accepting new patients and practices in the same office location you are accustomed to at 6 Woodland Rd, Unit 3b in Madison.

Once my practice is closed, your medical records will be securely stored according to legal standards. Future medical record requests can be sent to 6 Woodland Road, Unit 3b, Madison, CT 06443 Attn: Records. Requests will be filled as per time limitations dictated by state law retention requirements.
